| Upon
request to the Sequence laboratories
a DNA Collection Kit will be sent
to the address specified. |
The
Home DNA Collection Kit will
contain; |
- Mouth
swabs
- Instructions for taking
a mouth swab
- Individual
sample envelopes
- Test booking forms
- Informed
Consent Forms
- Terms & Conditions
documents
- Return addressed
envelope

Simply
follow the instructions for
taking the participants mouth
swab & fill
out the personal information
requested on the sample envelope.
You will be asked for a password
on the test booking form. This
will be assigned to your case
and will be asked for in any
telephone correspondence. |

Sign & Date the Informed Consent
Form as well as the Terms & Conditions
document. Return everything in
the addressed envelope provided. |
| Your
DNA test result will be available
within 5-7
days of the DNA Sample
Collection Kit arriving at
the laboratory. For Sibling
or Grandparent analysis the
turnaround time is 7-9
days. EXPRESS
SERVICES ARE AVAILABLE. |
