GP Collection Kits

Please note that: GPs may charge you for carrying out a DNA sample collection. This fee is payable directly to the GP you use. It is separate to the contract you have with this company.
The Process:
Upon request a GP Collection Kit will be sent to the address specified. Separate kits can be sent to multiple addresses if required.
The client should make an appointment with their chosen GP once the kit has been received.
Please note: Not all GPs will perform a DNA sample collection. GPs may also charge you for this service. Check these details with your chosen GP before ordering your GP collection kit and before booking an appointment.
The GP Collection kit will arrive sealed. It must remain sealed until the GP opens the kit at your appointment. If the kit has been tampered with, the GP will be instructed not to continue with the sample collection.
The GP DNA Collection Kit will contain:
- Test booking forms
- Instructions for taking a mouth swab
- Mouth swabs
- Individual sample envelopes
- Return addressed envelope
The GP will follow the instructions for taking the participants mouth swab & fill out the personal information requested on the sample envelope. The GP will also check test participant ID if the test is for legal purposes.
You will be asked to provide a password for the test booking form. This will be assigned to your case and will be asked for in any telephone correspondence.
You will be required to sign a legal declaration on the test booking form. This declaration concerns aspects of consent, test limitations and our terms and conditions. The information it refers to can be viewed under the ‘Documentation’ tab of this website.
Once completed, the GP will return everything in the addressed envelope provided.
Payment: Your payment should be returned using the payment form and return envelope supplied with the kit. Please note that a small deposit may be required when the kit(s) is despatched.
Your DNA test result will be available within 5-7 days of the DNA Sample Collection Kit arriving at the laboratory.
